Please leave a message with your name, phone number, reason for appointment, preferred times and dates (if applicable), and preferred languageA member of our team will reach out to you to confirm an appointment as soon as we can!

Bring to your appointment (if applicable):

  • A list of ALL your current medications (including any traditional herbal medications or supplements)

  • Any relevant medical records (e.g. X-rays, lab results, etc.)

Bringing these to your appointment will greatly aid our doctors in giving you the most comprehensive and accurate medical care.

To request Medical Records

Please come in person to fill out a medical release form to receive your medical records. We highly recommend completing this form (by request) during your appointment, as we cannot currently accept forms online.

Our Sister Clinics!

VN CARES is dedicated to serving the populations of the greater Sacramento community. We are proud to partner with VN Cares to offer Vietnamese translation services and free cancer screenings. 

H.L.U.B. Clinic aims to provide free healthcare services to the underserved Hmong Community. We are proud to partner with H.L.U.B. to lift the language and cultural barriers that exist between the Hmong Community and the Western healthcare system.

Other UC Davis Student-Run Clinics

  • Bayanihan Clinic offers primary and preventative healthcare for Filipino immigrants and all diverse, underserved communities of the Greater Sacramento area, while honoring the legacy of Filipino veterans.

  • Clínica Tepati provides basic healthcare services and health education to the underserved Latino community

  • Knights Landing Clinic provides linguistically competent and culturally humble healthcare services to the rural underserved in the Knights Landing area.

  • Joan Viteri Memorial Clinic offers unbiased health care to individuals who use intravenous drugs, work in the sex trade, or identify as transgender, as well as to their families

  • Shifa Clinic is focused on spiritual, socio-cultural, and language-specific medical care for every individual, irrespective of race, ethnicity, religion, and gender.

  • The Willow Clinic aims to holistically improve the health of community members who are currently living outside or experiencing homelessness.
